Dear Scholarship Applicant,
Congratulations on your decision to continue your education and to apply for an Illinois Destination ImagiNation scholarship. Before filling out your application please read and be sure you fully understand your responsibilities regarding the application process.
I.        Eligibility Requirements:
a. Applicant must have been active in Destination ImagiNation or OM Association Inc.for a minimum of two years, one of which must have been in a region of Illinois.
b. Applicant must be a graduating high school senior or a current college student.
c. Scholarship applies to all accredited two or four year colleges/universities and post-secondary vocational schools in the United States.
d. Scholarship is for a one-time award of $500.00.
II.        Notification
Up to two (2) awardees may be chosen at the discretion of the Committee. The scholarship recipient(s) will be notified by email, mail or phone. Formal announcement will be made during the awards assembly at the 2010 Affiliate Finals Tournament in April.
The money will be sent to the student upon receiving verification of their enrollment.
III.        To be eligible for an Illinois DI scholarship, complete the following steps:
o    Fill out the Online Scholarship Application Form. When you click on the "Submit" button, a Confirmation Page will open. Please print off 2 copies of the Confirmation Page. Copy #1 is for the Committee- sign across the bottom of the page. Remember that if you are under 18 years of age, your parent's signature is required too. Copy #2 is for your own records.
o    Submit a “Creative Essay” :
Create a presentation, viewable electronically and under 10 MB, that has value to some aspect of the Illinois Destination ImagiNation® Program
OR
In a creative way , convey one asset or attribute about yourself that was gained or enhanced through participation in Destination ImagiNation® programs. This piece may be expository, fictional, or poetic and should exemplify your style, creativity and abilities.
Your creative essay in whatever format you choose, must also be sent to the Committee in "electronic" form (Word document or PDF)
o    Submit two Letters of Recommendation in "electronic" form (Word document or PDF)
Please adhere to all guidelines when submitting your materials. Only complete applications will be considered.
Send electronic versions of the Creative Essay and Recommendation Letters to the Committee at scholarshipcommittee@illinoisdestinationimagination.org.
The deadline for all submissions of applications is February 28, 2010.
